Lately, we have had a lot of no show patients or last minute cancellations. We cannot fill these slots. Other patients may be waiting for months to see us. We will charge $50 for a new patient that no shows or cancels with less than 24 hour notice or $30 for a follow up patient with the same criteria.
Dr. Hahnke is in the Mesa office Monday through Wednesday, and in the Scottsdale office Thursday and Friday.

Dr. Perelman is in the Mesa office on Thursday and the rest of the week in Scottsdale.

The Mesa office is closed on Fridays.
We now have a continuous glucose monitor at each office. We can monitor blood sugars for 3 days and then download them. This can usually be billed through your insurance but we need to get a prior authorization first. However, patients/families will be responsible for the disposable sensor which costs $50.00. Let us know if you are interested in being monitored to help with control.
Please be aware that we charge a minimum $10 fee for any forms, paperwork, or letters that we are asked to complete or write for you or your child. This includes diabetes school orders, FMLA paperwork, airline travel letters, camp forms and other such forms.

As a courtesy to us, we request that all families who need forms completed should first fill out these forms themselves, as completely as possible. The doctors can then review the forms, complete any limited missing information, and sign them for you free of charge. If you choose not do so, you will be charged the paperwork fee. (NOTE: We do have blank diabetes school forms available for you to complete if your child?s school did not provide you with its own form.)
We are happy to respond to questions by e-mail. However, please note that this is not an appropriate communication medium for urgent or emergency kinds of problems. Please use the emergency phone line for this need.
Our practice functions on a very tight schedule and we strive to be on time for EVERY patient. Thus, we cannot tolerate late arrivals.

We will make every attempt to work you in on the date of your appointment, if possible, but we may need to re-schedule if you arrive after your appointed time.
